card_issuance(card_name, issue_month, issued_count)

For each card, find its earliest month in the table and return the percent change in issued_count from that launch month to the month that follows it in the data.

Return card_name, launch_month and month2_growth_pct rounded to two decimals, ordered by card_name. Cards with only one month are left out.

"The following month" means the next month present for that card, which is not always the next calendar month.
